Can Acupuncture Help the Brain "Reorganise" After a Stroke?

A new look at a landmark brain-imaging trial suggests acupuncture may help stabilise the brain's internal communication networks after stroke — and the researchers are admirably honest about what's still unproven.

A few days ago we wrote about a study showing acupuncture was linked to increased grey matter in motor-related areas of the brain after stroke. This week, a closer analysis of that same trial adds another layer — and it's one we think is worth sharing, because it shows exactly how careful, honest science actually works.

A different angle on the same trial

The study comes from the same randomised trial we covered previously: 56 stroke patients with one-sided paralysis (hemiplegia), split 2:1 between real acupuncture and sham (placebo) acupuncture over two weeks, with brain MRI scans taken before and after treatment in 46 of the participants.

This latest analysis looks at something different — not just which brain regions changed size, but how different brain regions talk to each other. Specifically, the researchers looked at the default mode network, a system of brain regions that stays active during rest and plays a role in attention, self-awareness and the background processes that help guide movement.

What the researchers found

In stroke patients, this default mode network often becomes disorganised — its different parts lose their normal coordinated rhythm. The researchers found that patients who received real acupuncture showed a more stable, less fragmented default mode network after treatment, and this change was linked to better motor recovery.

Alongside this, the acupuncture group again showed increases in grey matter volume — this time described more broadly as regions involved in sensorimotor processing and cognitive-motor integration, building on the earlier findings. Patients whose brain networks changed the most also tended to show the greatest improvements in movement.

What the researchers say still needs proving

Important caveats from the study authors

  • The sham (placebo) acupuncture group also improved — likely reflecting placebo effects and the standard stroke medication both groups received alongside the trial
  • The study did not find a statistically significant difference in how the two groups changed over time, despite the real acupuncture group showing larger average improvements
  • MRI changes in grey matter volume are markers of structural adaptation, not direct proof of cellular-level neuroplasticity
  • There was no long-term follow-up, so it remains unclear whether these brain changes translate into lasting functional benefits
  • The authors call for larger studies with longer follow-up before drawing firm conclusions

Why we're sharing this anyway

Some clinics only share research when it sounds dramatic. We think the more useful approach is to share research as it actually is — promising, early-stage, and part of a growing body of evidence that acupuncture has measurable effects on the brain, even when the full picture isn't settled yet.

What this study adds to the picture: acupuncture, alongside standard stroke care, is associated with brain changes that correlate with better movement — and it does so with an excellent safety profile and low cost. That's a reasonable basis for considering it as part of a stroke recovery plan, discussed with your medical team, while science continues to refine exactly how and for whom it helps most.
